You obtain Von Croy's Notebook in Margot Carvier's Apartment. Press Tab/Select to pause the game and view your inventory. Press Down to select 'Notebook,' Enter/X to open it, direction keys/D-Pad to turn pages, and Escape/Triangle to close. Note that new entries appear at the BEGINNING of the notebook, not the end. So turn pages to the left to see Lara's notes, to the right to see the entries Von Croy made before Lara got the book.
During gameplay, the notebook icon appears at the bottom right of the screen whenever a new entry is added. If you're stuck, check the notebook. In the remastered game, you'll need to collect and read all the notes to unlock the "Memento Mori" achievement/trophy. I have attempted to highlight all entries in this walkthrough, but I can't guarantee 100% until I've actually played the remastered game.

NOTE: These entries are listed, more or less, in the order in which they are obtained. The numbers in parentheses indicate the order in which they appear in the COMPLETED NOTEBOOK at the end of the game. For example, Von Croy's entries are already in the notebook when you get it, but they appear at the back of the book (entries 35 through 53). Similarly, you'll probably speak with the City Guide in the Parisian Ghetto before talking to Janice, but their entries appear out of order when paging through the completed notebook. Also, this guide is based on the original game. In the remaster, new entries have been added, and some entries with English and Latin translations have been merged. There are apparently 61 total entries, and you need to examine them as you discover them in order to unlock the achievement/trophy. I will continue to update as I go along.
