Below is a list of the 36 ways. Note that if you miss any, you can clean up later by loading earlier saves (with some caveats). You can also start a New Game or New Game+ after beating the game the first time, and the totals from your first playthrough should carry over. I'm not sure about Xbox and PlayStation, but Steam tracks your progress, although there is sometimes substantial lag before the counter updates.

ENEMIES:
NOTES ON SPECIFIC DEATHS: The bubbling lava in the Atlantis levels, where little bits of lava shoot up out of the surface of a lava pool, can kill Lara if she's exposed long enough, but this apparently doesn't count toward the achievement/trophy. Neither do the lava flows in the trap rooms in Atlantis.
*Several players have reported that being killed by rats while swimming does not count. So if you're missing one death, try loading a save from the beginning of the Cistern and letting a land-dwelling rat nibble Lara to death.
**Similarly, death by mutant may only count if they physically attack Lara, rather than killing her with fireballs/projectiles.
GENERAL NOTES: Some players have reported deaths not registering if they reloaded quickly, rather than waiting for the Passport menu to open on its own. I have not found this to be the case.
NOTES ON SAVE FILES: If you missed a death or two, you can reload different saves from the same playthrough, and even start a new game. The subsequent deaths should still count. You can also use the level-skip cheat and it won't void the achievement/trophy. However, if you have downloaded a save file or have backup saves stored in a folder, USB drive, etc., and replace your current save with one of those, the death count will be reset. I learned this one the hard way. :'(
